 We
participate in a number of insurance plans. For
those plans we submit the insurance claim form
and ask that you pay any required co-pay and for
any service that would not be covered by your
insurance-i.e. cosmetic procedures that are not
medically necessary as determined by your insurance
company.
If we are not participating providers with your
insurance company, payment is due at the time
services are rendered. We will provide you the
necessary Form to submit to your insurance company.
