Patients
are seen by appointment only. Please inform the
staff if you feel that you have an urgent problem
when you call to schedule an appointment.
If your insurance company requires a referral
please bring the referral or ensure that it is
in our office prior to the date of your appointment.
If you are a new patient, or have not had a visit
in the past year, please arrive at least 15 minutes
early to fill out a questionnaire and allow the
staff time to make a new chart for you.
If you need to reschedule or cancel an appointment
please let us know at least one business day ahead
of time.
